看图识字，是儿童学习文字的起始。与儿童一样，从感性到理性，通过看图谱学习皮肤病，可能是我和大多数皮肤科医生都走过的路。有了一定积累后，我特别想有一本罕见、少见、异形疑难的皮肤病图谱，以快速提升自己的诊断水平。很失望，我始终未能如愿。2005 年，为了培养研究生和年轻医生，我们启动了《罕见、少见、异形及疑难皮肤病图鉴》的编写，但实施中发现当时的积累远远难以达此目标，只好等待。2018 年在完成《实用皮肤组织病理学》一书后，我默默地做着准备，后因写作《白癜风》一书等其他工作暂时停顿。经过反复思考与查找图片等准备，2021 年9 月正式在科室全体医生中宣布启动工作，请近半年内有时间和精力的医生主动报名参与。该工作获得了大家的热烈响应，并进行多次充分讨论，王刚教授提议的书名《少见及疑难皮肤病图鉴》获得了认可。
本书编写初衷的目标读者是中、高级职称医师，筹备会上编委们认为有必要为每个常见病增加1 个典型病例，让初级人员也能学习，使年轻医生在掌握典型病例后能快速掌握疑难、少见及不典型的皮肤病，加速成长过程。
目录编排、分类是一本书的灵魂。本书的主目录根据疾病的好发部位编排，将罕（少）见与常见不典型皮肤病分开，每部分再按结节斑块、丘疹水疱、斑疹顺序，此编排有利于读者在临床工作中查找不认识的疾病。对一些临床形态变异特别大的疾病，我们特列了第八章，挑选了十大类疾病，适当多选一些病例，以便于读者系统性学习。此外，我们列出了发病年龄索引、特殊皮损形态索引，因其中列出的疾病超出本书而改为附录，相信这些附录和索引将为读者学习提供很大的便利。
书中大量的疑难病例选自临床诊断错误，由病理、化验等检查后修正者。这些罕见、少见、不典型皮肤病对于主治医师及低年资副主任医师有很大难度，图片挑选由临床经验丰富的高年资医师实施，经多位主编反复审定。初稿由年轻医生书写后，副主编、主编及主审轮番审修，因此本书最终形成了一个较大的主编、副主编及主审团队。正因每个病例均经过至少10 人次之手，作者的贡献最终根据承担的工作数量及质量体现在作者排序中。
本书的图片主要选自科室2005 年以来取病理时拍摄的百余万幅数码照片，基本上未用早年像素偏低的数码照片及以往积累的胶片照片。一些不需取病理的照片主要选自住院患者照片及部分医生平时积累的门诊照片。
毛发疾病一节特邀北京大学第一医院杨淑霞教授编写并提供全部图片；麻风一节特邀陆军军医大学西南医院杨希川教授审修并提供较多图片，后又特别邀请山东第一医科大学附属皮肤病医院张福仁教授把关；梅毒一节特邀上海市皮肤病医院王秀丽教授修改和补充病例；深部真菌病、诺卡菌病一节特邀四川大学华西医院冉玉平教授审理。部分病例通过临沂市人民医院陈洪晓教授建立的“天天向上”皮肤病理交流平台征集而得。全部非空军军医大学西京医院皮肤科的病例均已在文中注明提供者及单位。在此特向对本书做出贡献的全部同仁表示衷心感谢！
尽管所选图片由不同层次的医生挑选和把关，但限于一个独立单位的认知水平，仍难免有误诊误判或描述错误，衷心希望读者予以指正。
